Applicant Name (Prescreen) Block

Allows entering an applicant name on a prescreen form and provides a "People Finder," "Client Finder," or "Conflict Search" link depending on the configuration option selected.

The name fields displayed are for an individual. There is no option to enter a group client name.

Configuration Options:

* Ask for SSN

* Require SSN

* Ask for DOB

* Require DOB

* Show Partial Search Checkboxes

Search Method Options: People Finder, Client Finder or Conflict Search
