Applicant Phone #s Block

Can display several phone numbers on a form. When used on a profile page it displays all phone number fields that have a value.

Configuration Options

• Require at least one phone
• Show Home Phone
• Require Home Phone
• Show Business Phone
• Require Business Phone
• Show Mobile Phone
• Require Mobile Phone
• Show Fax Phone
• Require Fax Phone
• Show Other Phone
• Require Other Phone
• Show Phone Notes for all phone #s
• Require Ten Digits for all phone #s
• Show Additional Numbers - displays a button that shows all numbers not shown selected above
• Online Intake Format
• Do not format text entered - Useful if you have clients with non-US numbers
• Show "Safe?" checkboxes
• Preferred Type - dropdown to hide or display the Preferred Phone field, or use radio buttons to show and select preferred phone
• Default Preferred Phone
