Set Contact Type Block

This block is available for dynamic donation forms. It is designed to allow "Donor" to be automatically added to a donor's contact types when a donation from an individual is recorded.

It does not display any elements on a form except for an optional note.

Configuration Options:

* Show Note When Changing Contact Type - this option displays a note on the form indicating the selected contact type will added to the person's contact record.
