Disability- Medical History Block

Provides a means to record a disability entry for a case. Cases can have multiple entries. The fields displayed are determined by the configuration options selected.

This block is typically placed on a form below either the "Disability-Medical History" list view or the "List Disability-Medical History (For Editing)".

* Show date problem started

* Show date problem diagnosed

* Show medications - text box

* Show nature - text box

* Show physicians/hospital - text box
