DAP V2 Block

Displays a dropdown DAP Outcome field with options for SSI Benefits Received, DIB Benefits Received, or Both SSI and DIB Benefits Received. Depending on the selection, displays one or both sets of the following fields, which are dollar amounts:

* SSI Monthly Outcome

* SSI Retro Outcome

* DIB Monthly Outcome

* DIB Retro Outcome

It then displays calculated amounts for DAP - Monthly Total and DAP - Retro Total.

Finally it displays an editable Outcome - Interim Benefits dollar amount field.
