Private Bar Referrals Block

This block displays a list of existing private bar referrals, and allows you to add a private bar referral.

It displays the following fields for adding a referral: Referral Date (defaults to today), a required Referred By dropdown (defaults to the current user), a Status dropdown, Amount Collected (from the private attorney), the date the money was collected from the attorney, and a text box for notes.
